Universal Grade Change Form

I created this online version of one of the funniest things I've ever read. It's called the "Universal Grade Change Form" and I got it from my 12th Grade Social Studies teacher. It's also available here. It's hysterical and now you can actually create the form online and (obviously optionally) e-mail it to a professor! Make sure he/she has a good sense of humor first, though.
